Eddy SeniorCare/PACE (Program of All-Inclusive Care for the Elderly) is a unique, integrated medical and long-term healthcare management program designed to help chronically ill adults live independently in their own community and avoid unnecessary nursing home placement.
Eddy SeniorCare is modeled after the established PACE national service model first developed as On Lok in San Francisco, CA, which has been operating across the country for more than two decades.
Eddy SeniorCare offers an interdisciplinary care team -- comprised of a physician, nurse, social worker, therapy professionals, pharmacist, day center staff, home care and transportation personnel -- all working together to manage each seniors' medical conditions. Services include: day care, physician, nursing care, rehabilitation, medical specialties, social work, transportation, recreation, meals, personal care and in-home services.
All healthcare services are provided through Eddy SeniorCare, including home and hospital care, and even if you experience a decline in your health and should require nursing home placement. Eddy SeniorCare does not disenroll individuals on the basis of health care status. In any environment, the program will continue to care for participants for the remainder of their life.
Eddy SeniorCare also offers a variety of health and educational programs for clients and their families and the community, including a fall prevention program, a post-stroke group, and a caregivers' support group.
